We have a couple of ways to get your New Patient Intake paperwork started.
- Call the office to schedule your 1st appointment. The Wellness Coordinator will send you a link to complete the paperwork online, on an encrypted server.
- If you would prefer to complete this on paper, please click here for new patient paperwork.
Please fill out the new patient form as completely as possible and bring it with you to your initial visit.
For your appointment, wear comfortable, loose-fitting clothing and please do not wear perfumes or colognes in case another patient is chemically sensitive.
